Ochrona danych osobowych
Wyrażam zgodę na przetwarzanie moich danych osobowych (Dane) przez Fundację Rozwoju Systemu Edukacji w Warszawie, przy Al. Jerozolimskich 142A (Fundacja), do celów związanych z organizacją przez Fundację niniejszego, o który mowa przy niniejszej czynności rejestracyjnej (Wydarzenie) i moją obecnością na Wydarzeniu zgodnie z Rozporządzeniem Parlamentu Europejskiego i Rady UE 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (ogólne rozporządzenie o ochronie danych). Danymi mogą być moje imię oraz nazwisko, adres poczty elektronicznej, nazwa instytucji, mój image (twarz). Przyjmuję do wiadomości, że Fundacja jest administratorem Danych, które pozyska w związku z moim udziałem w Wydarzeniu. Zgoda, której niniejszym udzielam na przetwarzanie Danych, jest dobrowolna i swobodna. Zostałem poinformowany / zostałam poinformowana, że mam prawo do cofnięcia zgody na przetwarzanie Danych w całości lub w zakresie wybranym w każdym czasie w każdy możliwy do zidentyfikowania i uchwycenia sposób. W razie jakichkolwiek wątpliwości Fundacji odnośnie zakresu oświadczenia o cofnięciu zgody na przetwarzanie Danych, Fundacja będzie przyjmowała wykładnię rozszerzającą takie oświadczenie, ażeby jak najlepiej chronić moje interesy oraz Dane, wglądu w Dane, żądanie ich poprawiania, weryfikacji, przenoszenia a także prawo do żądania ich usunięcia bez zbędnej zwłoki, co jednak nie wpływa na fakt i zgodność z prawem przetworzenia Danych przed taką moją decyzją, wniesienia skargi do organu nadzorczego: Generalny Inspektor Ochrony Danych Osobowych (od 25.05.2018: Prezes Urzędu Ochrony Danych Osobowych), ul. Stawki 2, 00-193 Warszawa, gdy uznam, że przetwarzanie Danych jest niezgodne z prawem. Dane będą przetwarzane przez okres mojego udziału w Wydarzeniu, a także przez czas do maksymalnie 3 lat (w szczególności dla celów archiwizacyjnych, ewaluacyjnych, kontrolnych i statystycznych), z zastrzeżeniem Danych stanowiących image. Dane będą przechowywane w siedzibie Fundacji, z możliwością wykorzystania wirtualnych i elektronicznych zasobów i źródeł magazynujących. Fundacja wyznaczyła osobę odpowiedzialną za zapewnienie przestrzegania przepisów prawa w zakresie ochrony danych osobowych, z którą można skontaktować się pod adresem e-mail: iod@frse.org.pl.

Use of Image
As an adult with full legal capacity, I hereby consent to the use by the Foundation for the Development of the Education System, with its registered office in Warsaw at Al. Jerozolimskie 142A (hereinafter: the “Foundation”), of my image (face, clothing, behavior, voice, statements, manner of expression), which the Foundation may record in the form of photographs and audio-visual works during events carried out with the participation of the Foundation related to its statutory activities. I grant this consent free of charge and without any time or quantity limitations. The consent covers recording, editing, modifying, reproducing materials containing my recorded image and their dissemination via any medium (any form of transmission of image and sound), in accordance with the purpose and activities carried out by the Foundation, in particular for promoting programs (projects) conducted by the Foundation, without the need for prior approval. The Foundation is also entitled to share materials containing my recorded image, as well as my personal data necessary for identification purposes (first name, last name, institution), with entities cooperating with the Foundation for the purposes of promotion, use, and dissemination of the results of the Foundation’s activities. I further declare that the use of my image in accordance with this consent does not infringe my personal rights or any other rights.
By granting the above consent, I acknowledge that:
1. The controller of personal data (including the image processed on the basis of this consent) is the Foundation for the Development of the Education System (“Controller”).
2. Personal data will be processed solely for the purpose specified above in this consent.
3. My image will be processed for the period specified above in this consent.
4. The data are protected by technical and organizational measures to ensure an appropriate level of protection, in accordance with applicable regulations.
5. The Controller will process my image for the purpose described above on the basis of Article 6(1)(a) of the GDPR and does not obtain data from third parties.
6. The data may be shared with: a. entities providing IT services, accounting services, administrative services, or other services related to the performance of services for the Controller; b. institutions managing programs implemented by the Foundation for the Development of the Education System, to the extent necessary to carry out those programs.
7. You have the right to: obtain information about the processing of the Data, including categories of processed data and potential recipients; request correction of inaccurate Data or completion of incomplete Data; request deletion or restriction of processing – under the rules specified in the GDPR; object to processing – under the rules specified in the GDPR; data portability – by receiving the Data from the Controller in a format enabling transfer to a selected third party; lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office, ul. Stawki 2, 00‑193 Warsaw – if you believe the Data are processed unlawfully.
8. You have the right to object to the processing of all or some of the Data at any time. The objection does not affect the lawfulness of processing carried out before it was made. The objection may be submitted by sending a statement to the Foundation for the Development of the Education System, Al. Jerozolimskie 142a, 02‑305 Warsaw, or by email to: iod@frse.org.pl
9. Any requests, questions, and demands related to the processing of the Data should be addressed to: iod@frse.org.pl.

I declare that I meet the formal requirements for participation in the event and that:
I have an account on the ESEP platform and it is active and fully functional.
I have an account on the ESEP platform, but I have problems logging in or the account is not fully functional (in case of issues with the account, please contact the eTwinning helpdesk: 728 455 326, etwinning@frse.org.pl).
I do not yet have an account on the ESEP platform, but I will create one within 24 hours after submitting the form. I understand that the lack of an active account will result in removal from the list of qualified participants.
