CONSENT and DECLARATIONS

I have read the information about the processing of personal data
In accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, hereinafter “GDPR”, Official Journal of the EU L 2016, No. 119), we inform you that:
1. The controller of your personal data is the Foundation for the Development of the Education System (“Controller”).
2. Personal data will be processed for the purpose of ensuring your participation in the activity described in the introduction to this notice.
3. For the purpose indicated in point 2 above, the following personal data are processed: first name, last name, email address, institution (“Data”).
4. The Data will be stored by the Controller until the completion of the settlement of the service implementation and for the period required by law, to the extent necessary to comply with legal obligations, or until you object to the processing of the Data.
5. The Data are protected by technical and organizational measures to ensure an appropriate level of protection, in accordance with applicable laws.
6. The Controller does not obtain Data from third parties or publicly available sources and processes only the Data provided by you for the purpose described in point 2 above (in accordance with Article 13 of the GDPR).
7. The Data may be shared with:
a. entities providing IT services to the Controller or entities providing accounting services, administrative services, or other services related to the performance of the service;
b. trainers to the extent necessary to deliver the training service;
c. institutions managing programs implemented by the Foundation for the Development of the Education System, to the extent necessary to carry out those programs;
d. your employer (if you have been delegated to participate in the event specified in the header by their decision).
8. The legal basis for processing the Data is Article 6(1)(b) and (f) of the General Data Protection Regulation of 27 April 2016 (GDPR), and, with regard to information provided to the European Commission within programs managed by the European Commission and its agencies – Article 5(1)(c) of Regulation 2018/1725.
9. You have the right to:
a. obtain information about the processing of the Data, including the categories of data processed and potential recipients;
b. request correction of inaccurate Data or completion of incomplete Data;
c. request deletion or restriction of processing of the Data – under the principles set out in the GDPR;
d. object to processing – under the principles set out in the GDPR;
e. data portability – receive the Data from the Controller in a format allowing transmiss ion to a chosen third party;
f. lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office (UODO), ul. Stawki 2, 00-193 Warsaw – if you believe the Data are processed unlawfully.
10. You have the right to object to the processing of all or some of the Data at any time. The objection does not affect the lawfulness of processing carried out before it was made. Use of Image
As an adult with full legal capacity, I hereby consent to the use by the Foundation for the Development of the Education System, with its registered office in Warsaw at Al. Jerozolimskie 142A (hereinafter: the “Foundation”), of my image (face, clothing, behavior, voice, statements, manner of expression), which the Foundation may record in the form of photographs and audio-visual works during events carried out with the participation of the Foundation related to its statutory activities. I grant this consent free of charge and without any time or quantity limitations. The consent covers recording, editing, modifying, reproducing materials containing my recorded image and their dissemination via any medium (any form of transmission of image and sound), in accordance with the purpose and activities carried out by the Foundation, in particular for promoting programs (projects) conducted by the Foundation, without the need for prior approval. The Foundation is also entitled to share materials containing my recorded image, as well as my personal data necessary for identification purposes (first name, last name, institution), with entities cooperating with the Foundation for the purposes of promotion, use, and dissemination of the results of the Foundation’s activities. I further declare that the use of my image in accordance with this consent does not infringe my personal rights or any other rights.
By granting the above consent, I acknowledge that:
1. The controller of personal data (including the image processed on the basis of this consent) is the Foundation for the Development of the Education System (“Controller”).
2. Personal data will be processed solely for the purpose specified above in this consent.
3. My image will be processed for the period specified above in this consent.
4. The data are protected by technical and organizational measures to ensure an appropriate level of protection, in accordance with applicable regulations.
5. The Controller will process my image for the purpose described above on the basis of Article 6(1)(a) of the GDPR and does not obtain data from third parties.
6. The data may be shared with: a. entities providing IT services, accounting services, administrative services, or other services related to the performance of services for the Controller; b. institutions managing programs implemented by the Foundation for the Development of the Education System, to the extent necessary to carry out those programs.
7.
